橋本病とIgG4関連硬化性疾患

1 . はじめに

　橋本病は1912年日本人外科医橋本策により初めて発見された甲状腺の炎症性疾患である¹⁾．リンパ球浸潤という病理組織学的特色から“struma lymphomatosa”と命名されている¹⁾．橋本病は，甲状腺の線維化，濾胞上皮細胞の好酸性変化と委縮などを病理組織学的特徴とする自己免疫性炎症性疾患で，通常は内科的に治療され，経過は良好とされている²⁾．また橋本病には，予後の良い大多数と，甲状腺機能低下症に急速に進行する例や，痛みのため外科的対応を必要とする例が少数あることが知られている^3,4)．その差を規定する因子や病態の差を説明する知見は，最近まで知られていなかった．

　一方，甲状腺におけるIgG4関連硬化性疾患(IgG4-related sclerosing disease；IgG4RSD)はリーデル甲状腺炎と推定されていた⁵⁾，しかし筆者らの経験したリーデル甲状腺炎の1症例では，IgG4陽性細胞の増加を確認することができなかった(未発表データ)．

　本稿では，橋本病，亜急性甲状腺炎，リンパ球性甲状腺炎の手術例17例のIgG4免疫染色結果と血清IgG4測定が可能であった5例を含む甲状腺手術例70例の解析から明らかになった橋本病とIgG4RSDの関連について報告する⁶⁾．
