1 . はじめに

　IgG4関連疾患は高IgG4血症と組織でのIgG4陽性形質細胞の浸潤と線維化病変を特徴とする炎症性疾患であり，本邦での自己免疫性膵炎(autoimmune pancreatitis；AIP)とミクリッツ病(Mikulicz disease；MD)における血清IgG4上昇の発見から明らかになり始めた^1,2)．それらに合併するかたちで多臓器の障害が次第に報告されるようになり，現在では，腎，肺，大動脈(後腹膜)，下垂体，甲状腺，前立腺，副鼻腔，皮膚などに時間的空間的多発性をもった全身性炎症性疾患群になりつつある．ここでは，特に鑑別が重要な疾患群を列挙し，その共通点や差異を述べる．
