血液中のトランスフェリンに結合しないフリーな鉄を総称して非トランスフェリン結合鉄(NTBI)と呼ぶ.このNTBIは生体にとって有害となる鉄の存在形式で,その測定は臨床的に鉄が生体に及ぼす影響をとらえるうえで重要と考えられている.筆者らはnon-metal HPLCを用いた安定かつ高感度なNTBI測定法を確立し,より正確な健常人の血清NTBI値を算出することに成功した.これを基準に,体内の鉄動態を把握する新しい指標としての血清NTBI値の可能性について,これまでに報告された病態とNTBIとの関係も含めて考察する.
