肝移植におけるADAMTS13活性測定―周術期におけるADAMTS13活性モニタリングの意義

■近年，肝移植後の致死的合併症の1つとして血栓性微小血管障害（TMA）が注目されている．

■UL-VWFMの特異的切断酵素であるADAMTS13は主に肝星細胞で産生されるため，肝移植後早期にはADAMTS13活性が著明に低下する．

■TMAの治療には早期診断が極めて重要であるが，周術期にADAMTS13活性をモニタリングすることにより，肝移植後のような凝固線溶能がダイナミックに変動する病態であってもその推移をリアルタイムに把握し，適切な治療を施行することが可能になるものと期待される．
