ヘパリン起因性血小板減少症の診断における抗血小板第4因子/ヘパリン複合体抗体測定の意義

　ヘパリン起因性血小板減少症（HIT）の検査薬として，専用の全自動分析装置に用いる3種類の製品が承認された．化学発光免疫測定法として，ヒーモスアイエルアキュスターHIT-Ab（PF4-H）とヒーモスアイエルアキュスターHIT-IgG（PF4-H）の2種類がある．また，ラテックス凝集法として，ヒーモスアイエルHIT-Ab（PF4-H）がある．臨床現場からの3種の測定結果に関する報告はわずかで，そのHIT診断に対する有用性は十分に解明されていない．ただ，ベットサイドで簡便に結果が得られるゲル粒子免疫測定法やイムノクロマト法と同様に，その迅速性の点は評価されている．しかし，3種ともに測定結果は数値単位（U/mL）として印字されるが，その単位の高低に関する臨床的意義は知られていない．ヘパリンの使用中の血小板減少や血栓の発生といったHITを十分に疑うに足る根拠がない限り，3種ともに検査の適応とはならない．すなわち，“念のため”として安易に検査を実施すると偽陽性結果に悩まされることになる．現在，その臨床的有用性に関して，3種の測定法ともにHITの疑い例のHIT除外診断に有用であるとのコンセンサスが得られている．
