症例から学ぶ後天性von Willebrand病—大動脈弁狭窄症に合併する後天性von Willebrand病

Point

●大動脈弁狭窄症患者に貧血を認めれば，後天性von Willebrand病によって消化管angiodysplasiaからの出血を生じるHeyde症候群を疑う．

●Heyde症候群を疑えば，von Willebrand因子マルチマー解析を行い，高分子マルチマーの欠損を確認する．

●Heyde症候群によって消化管出血による重症貧血を生じていても，大動脈弁置換術や経皮的大動脈弁留置術（TAVI）治療によって治癒が期待できる．
