COPDの治療

Point

●COPDの原因はわが国の場合ほとんどが喫煙によるものであり，COPD患者を減らすためには専門医だけでなく，一般診療においても健康に対する喫煙の影響を説明し，禁煙を積極的に助言する必要がある．

●患者の状態に応じて積極的な治療介入が必要であるが，中心となるのは長時間作用型気管支拡張薬である．治療開始したら臨床的な効果を評価し，その治療が適しているかを判定することが必要である．

●吸入ステロイド薬（ICS）は気管支喘息合併例（ACO）で使用すべきで，重症度の高いCOPD患者では肺炎のリスクが高まるとされている．

●非薬物療法として，禁煙，ワクチン接種，呼吸リハビリテーションが重要であり，日常生活における身体活動性を高めることが生命予後の改善につながる．
