BNP/NT-proBNP—バイオマーカーを理解する

●B型ナトリウム利尿ペプチド（BNP）は心保護作用を有するペプチドで，ナトリウム利尿ペプチドファミリーの1つである．心不全などにおいて心負荷とともに心室から分泌されて血中濃度が上昇する．
●BNPとN末端プロB型ナトリウム利尿ペプチド（NT-proBNP）はいずれも心不全の診断，管理を行ううえで欠かせないバイオマーカーである．心不全の予後を予測する際にも非常に有用である．
●BNPとNT-proBNPには生理的作用，測定条件，バイオマーカーとしての測定意義，その値を解釈するうえでの注意点など異なる部分がある．それらの違いを理解する必要がある．
●心不全管理中のBNPやNT-proBNP値は過去との比較が大切である．それぞれの症例に最適な値をみつけ，その値を維持するような生活習慣の是正や適切な薬物療法などの包括的管理を行うことが大切である．
