僧帽弁逆流症

基礎知識

　僧帽弁逆流症（mitral regurgitation：MR）の機序は，僧帽弁弁尖，腱索，乳頭筋，左室や左房の異常であり，これらが単独，または複合して生じる．原因疾患はさまざまであり発展途上国ではリウマチ性のMRが多いが，先進国ではリウマチ熱の減少により頻度は少なく，僧帽弁逸脱症や高齢者による弁変性から生じる器質性MRが多い．症状は息切れ，浮腫，呼吸困難を主訴とし，無症状であれば健診での心雑音の指摘や胸部X線での心拡大，心電図異常を契機に発見されることが多い．身体所見として，左室拡大に伴い心尖拍動が左下方向へ偏位することや，全収縮期雑音とⅢ音が特徴的であり，収縮後期に逆流が増大する僧帽弁逸脱例では，収縮後期に強くなる全収縮期雑音や収縮後期雑音を呈する．また収縮期クリックは僧帽弁逸脱の特徴的所見である．
