基礎知識

　心不全とは，“心臓に器質的および/あるいは機能的異常が生じて心ポンプ機能の代償機転が破綻した結果，呼吸困難・倦怠感や浮腫が出現し，それに伴い運動耐容能が低下する臨床症候群”と定義される^1）．

　心不全の原因疾患は多岐にわたり，左室駆出率（left ventricular ejection fraction：LVEF）が40％未満であればHFrEF，LVEFが50％以上のものをHFpEFと分類し，その境界となるLVEF 40〜50％のものをHFmrEF（heart failure with mid-range ejection fraction）と呼ぶ．LVEFの保たれたHFpEFもHFrEFと生命予後に大きな差がないという結果が出ており^2），HFpEFの診断は重要となる（OPA①）．
