がん関連血栓症（CAT）

Point
●担がん患者は，がんおよびがん治療の影響で凝固亢進状態となり，がん関連血栓症（CAT）を発症するリスクが高い．がん患者を診察する際にはCATの存在を意識する必要がある．
●CATは浮腫や下肢の痛み，呼吸苦など症候性に発見されるだけではなく，造影CTなどの画像検査で無症候性に偶然，発見される場合もある．
●化学療法前のCATリスク評価としてさまざまなスコアリングが提案されているが，実臨床では個々のがん患者を注意深く観察し，Dダイマーの経時的変化や画像検査を併用して評価している．
●CATに対する抗凝固療法は，がん患者の状態や出血リスクなどを考慮したうえで，ヘパリン〔わが国では未分画ヘパリン（UFH）〕，直接経口抗凝固薬（DOAC），ワルファリンから選択される．
