臨床検査を用いた肝硬変患者における肝予備能の評価

はじめに

　肝硬変は，肝臓全体に再生結節が形成され，これを線維性隔壁が取り囲む病変と定義され，肝疾患の終末像である．肝機能がよく保たれ臨床症状がほとんどない代償性肝硬変と，肝性脳症，黄疸，腹水，浮腫，出血傾向など肝不全に起因する症状が出現する非代償性肝硬変に分類される．肝臓の重要な役割に，タンパク質などの合成と体内の老廃物の解毒がある．肝硬変では，肝細胞が慢性的かつ持続的に障害されることにより，肝実質細胞が減少し肝細胞機能不全となり，タンパク・アミノ酸・アンモニア代謝や糖・脂質代謝異常，ビリルビン・胆汁酸代謝の低下が起こる．その結果，アルブミン，総コレステロール，プロトロンビン時間（prothrombin time：PT），コリンエステラーゼなどが低下し，高アンモニア血症，高ビリルビン血症を呈するようになる．

　肝予備能は，これらの肝予備能を表す検査値や臨床所見を組み合わせて総合的に評価する．肝予備能の評価は，肝硬変患者の重症度の評価だけでなく予後予測にも有用である．

　本稿では肝予備能の評価法について解説する．
