T/NK細胞腫瘍1—成熟T/NK細胞白血病/小児EBV陽性T/NK細胞増殖症およびリンパ腫

Point

●成熟T/NK細胞白血病には，T細胞前リンパ球性白血病（T-PLL），T細胞大顆粒リンパ球性白血病（T-LGLL），NK細胞大顆粒リンパ球白血病（NK-LGLL），成人T細胞性白血病/リンパ腫（ATLL），Sézary症候群（SS），急速進行性NK細胞白血病（ANKL）がある．小児EBウイルス（EBV）陽性T/NK細胞増殖症およびリンパ腫は，慢性活動性EBV疾患（CAEBVD）と小児全身性EBV陽性T細胞リンパ腫（SEBVTCL）の2つに分けられる．

●T-PLLはTCL1AまたはMTCP1再構成やTCL1A蛋白発現，T-LGLLはSTAT3変異，NK-LGLLはSTAT3やTET2変異が診断基準として加えられた．

●ATLLには免疫回避に関与する遺伝子変化が，ANKLにはJAK/STATやRAS/MAPK経路の遺伝子，エピジェネティック修飾因子や免疫チェックポイント分子の変異が病因や病態に関与することが明らかになった．

●CAEBVDはTまたはNK細胞の両者に由来する可能性がある．CAEBVDには限局性でインドレントな重症蚊刺アレルギーと古典型種痘様水疱症リンパ増殖異常症（HV-LPD），システミックな病態として全身性には全身性HV-LPDや全身性CAEBVDがある．
