●肥満と運動器疾患との間には深い関連があり,特に変形性関節症(OA)の発症機序には体重増加による関節への荷重負荷の増大が関与していると考えられてきた.
●肥満は手関節などの非荷重関節の関節症発症にも相関しており,OAの発症には荷重負荷の増大以外のメカニズムがあると考えられるようになった.
●OAの発症には,肥満による“軽微な慢性炎症”が深く関与している.
●その軽微な慢性炎症には,脂肪細胞が分泌するサイトカイン(アディポカイン)が関与する経路と,脂質異常症やインスリン抵抗性による全身的な炎症が関与している.
